Professional Development Course – Setup Form
If you have developed a new course or activity, or have a repeat course that you want reviewed for professional development credit eligibility, please complete and submit the online Professional Development Course Setup Form. Or you may fill out a savable PDF version of the Course Setup Form and email it to firstname.lastname@example.org.
Once you’ve submitted the online form, you will receive a confirmation email with all of your submission contents. The Boise State K-12 Professional Development manager will send instructions after the course has been approved by the College of Education. Please be aware that depending on the time of year (end or beginning of semesters) the approval process may take up to two weeks. Please submit course requests as early as possible.
Professional Development Instructor Approval Form
If you are seeking approval to serve as an instructor for K-12 professional development courses, please complete and submit the online Professional Development Instructor Form. Once you’ve submitted the form, you will receive a confirmation email with all of your submission contents. The Boise State K-12 Professional Development coordinator will get in touch with you for finalization of the approval.
Professional Development Purchase Order (PO) Payment Form
If you previously set up a course to be offered for credit and did not provide PO information at that time, please submit the PO Payment Form to make arrangements to pay for credit registration fees. Once this form is processed, the credit registration page for the course will allow PO-paid registrants to indicate that they are included.
