Mountain Home is a military installation, which requires Boise State employees and students to submit to a security check before being granted access to the base.
Non-Military students attending classes at Mountain Home Air Force Base must submit to a background check before being allowed access to the base. Note: Background Check Forms must be submitted for each semester you are attending classes on base.
Students wishing to have access must submit the form at least two weeks prior to the start of class.
- Download a Background Check Form (PDF opens in new window), or contact the Mountain Home coordinator for a form. Fill in Items 1-6 within the box and and save the PDF onto your desktop.
- Once the Form is completed and saved, either:
- E-mail to: firstname.lastname@example.org
- Mail to: Boise State Mountain Home AFB Program, 665 Falcon St., Mountain Home, Idaho 83648
This procedure must be completed each semester you attend class on the Mountain Home Air Force Base.
